n-400 interview cancelled

I received an appointment letter for Naturalization interview. I went to the scheduled appointment and after 4 hours of waiting they told me that they haven't received my Permanent Immigrant File and therefore they can not interview me. Instead they told me to wait until they receive it and they will send me another appointment letter within 3 months.

1. Has this ever happened to someone else?
2. What is a Permanent Immigrant File?
3. Does this indicate a problem with my application?

n-400 time line:
04/29/04 - filed the application in California
06/17/04 - fingerprinted
09/29/04 - received an appointment letter for interview
11/15/04 - interview was cancelled
