A Motion to reopen or reconsider an application denied

I received a leeter from the USCIS saying "...A Motion to reopen or reconsider an application denied..." is there a special form or letter for the motion to reopen? I filled the I-539 (It was approved twice since 2004). I am here for medical treatment (I am 50% handycap), any advise on what to do next? Thanks

Doesn't look promising. You should secure assistance from a competent immigration attorney.
