What it basically does is captures your personal information like DL#, SSN, DOB, Name, Address etc and then at the end says print the attached PDF and mail it to your registrar along with the 2 evidence providing you are US citizen and proving your residence.
If one is intelligent that person can easily get this forms from either local library or DMV and hand write the information and send it in.
