You almost never ever give / submit original document to USCIS in any case...they write it clearly ... even if they want to see / look at original document ..they request you / you show them at the time of interview ... I haven't seen any form for such purpose coz you don't submit original document at all...call the national customer service center and talk to an immigration officer or go to your local uscis regional office and explain your situation ...good luck...Pasha
Dont ever file any original document in future..
