filed I-485 in feb99,had interview, finger prints and medical in 2002,we have obtained several advance parole to go out of the country,everytime we ask for an update on the adjustment of status, we are told is pending,then we have to wait 90 days between inquires,we filed an ead form i 765 by mail,we just received a response saying this request has been denied because service records do not show having a form i-485 pending for adjustment of status

You should check on the status of your I-485 application directly with BCIS customer service. If the I-485 has been approved, BCIS will not issue another EAD card, because there no longer is a I-485 "pending". At my old firm, that was the most common reason why we would get rejections of I-765 applications, which would then require us to track down the missing I-485 approval notices.
It's also possible that BCIS "lost" the file; however, if you have proof of filing, multiple EADs and APs, you can prove that the I-485 application exists.
