CHECK YOUR FORM. THE FITH NUMBER IS DEFAULTING TO THE SIXTH NUMBER. MAYBE IT'S JUST MY PROGRAM, BUT PLEASE CHECK

May be there is problem with the Form itself. Because all these forms are edited with Adobe Acrobat and each & every column has to be edited indivitually and alloted separate or distinguished numbers. It needs lot of concentration and time to compose a single Form.
If you still have problem and you can not edit yourself, then leave the rest of that column blank and after printing it, you can fill it in by hand or Typewriter.

Hey dude, mine does that too...i had printed it out about 6 times only to find out it was the wrong number, so instead i left that one and filled it in by hand..

Why not invest into a copy of Acrobat? Then you can manipulate the forms to make text fit, etc. I've done it, and LOVE it!

If you're broke and can't afford the approximate $200 price tag, then there's plenty of warez sites where you can download a copy.

You can download for free from www.kaaza.com. Make sure when you download Adobe Acrobat 5.0 Full Version, you must remove any version of Acrobat you have on your computer including Acrobat reader. From Kaaza you can download any program, movie or songs and even can make copies of CDs and DVSs.

I have the 5.0 version but im not sure if it is the full version or not. I cannot adjust the boxes on the form, so does that mean its not a full version??

oh it says Adobe acrobat reader 5.0?? thats what i have. Hmmm..looks like i will have to remove that one huh..

In addition to having a Adobe Acrobat full version, you also have to know how to use it. You can check in your program folders whether you have full version or only Reader.
