    I would use the word "approximately" when they ask how many times you've been to usa. You can explain what you mean when you go the interview @ embassy.

    State "frequent visits alloted within the law and appropiate according to the visa type" then add another piece of paper with as many accurate dates you can find, type of visa, purpose of visits etc.
