You can usually find a Notary Public at a bank or post office or in the yellow pages of phone book. Don't sign the form until you are with them. Take forms of ID. They will stamp the form indicating that they saw you sign the form in front of them and that you showed them ID proving that you are who you say you are. There may be a small fee.
