My company has never written a check to BCIS, the accounting department is asking me for a W-9 form from BCIS.

Have any of you tried to get a W-9 form from BCIS? Can you help me? Is there a tel# to call?

Does the check for I-129 have to come from the company? Can it come from me (alien) or the applicant (my manager)?
